If you would like to start the application process, please use any of the forms/applications on this page. They are all fillable PDF forms, so you can fill them out and print them, print the form, and fill out by hand, or fill out the form, save and upload through our secure upload link.
Secure Upload
When you click on the upload link, you will be directed to Secure Email Message Center. If this is your first time using our secure email, you will need to register. A confirmation email will be sent to your email once you register. When you click on the link and sign in, please click on the compose email. If you have already been in correspondence with a loan, please send the application/form directly to them. All other applications/forms should be sent to CSRMain@citizbank.com. You will receive a confirmation email within one business day.
